Am I Eligible for Medical House Call Services?

If any of the following apply to you, a loved one, or someone in your care, contact us to see how we may be able to help.

  • Intellectual and developmental disabilities
  • Need special transportation such as wheelchair accessible van
  • Use of assistive devices, such as cane or walker
  • Difficulty walking
  • Shortness of breath with exertion, Oxygen use
  • Multiple hospitalizations / ER visits
  • Difficulty with activities of daily living
  • Difficulty leaving home
  • Require someone to accompany you to appointments
What Insurance Do You Accept?

We accept Medicare, Medicaid, Keystone First, Keystone 65, Keystone VIP, Keystone Health Plan East, Independent Blue Cross, Aetna, Cigna, United Health Care, Ameri-health, Blue Cross Blue Shield, Humana and AARP Medicare.

 

What If There Is Another Doctor’s Name on My Insurance Card?

You will need to call the customer service number for your insurance company and ask them to change your PCP to our office. If you are not sure how to do this, our staff can assist you.

Will My Insurance Cover for a House Call Visit?

Medicare and Medicaid insurance will cover in home care for those patients who qualify. Medicare will cover 80% of the allowed amount with the other 20% billed to the patient’s secondary insurer, the same as the physician’s office. Deductibles or co-payments are required for medical services and are handled in the same way as a trip to a physician’s office.

Scheduling A Visit?

You can request for an appointment by clicking here, this will take you to our scheduling page. You will need to choose the date and time of desired appointment. The office will respond by sending you the patient intake forms. You will need to complete all new patient forms and consents prior to your appointment. You’ll also need to provide your insurance card to verify eligibility. You will be notified in 1-2 days of the expected time of arrival for your appointment as this is based on geographic location and daily schedule demands.

Will I see a Nurse Practitioner or a Physician?

All of your care is provided by board Certified Adult Nurse Practitioner. We also collaborate your care with a physician and do periodic chart reviews. A nurse practitioner can:

  • Diagnose and treat acute and chronic conditions
  • Prescribe medication and other treatments
  • Order and interpret diagnostic tests such as lab and radiology results
  • Serve as a primary care provider (PCP), manage a patient’s overall care
  • Educate patients and families on disease prevention and health and lifestyle choices
Can I Still See My Regular Doctor?

Medical House Call Clinic (MHCC) will assume the role of your Primary Care Provider. MHCC will communicate and coordinate with any specialists you have as deemed necessary.

 

How Quickly Can I Get an Appointment?

For new patients, home visits can typically be scheduled within 3-5 business days of the registration process being completed. In most cases, if you have Medicare, you should be scheduled for a visit within 24 – 48 hours from the time we are able to verify your insurance. Some HMO’s may not initiate a change in your PCP until the first of the following month, so that could require a longer wait.

 

Are Weekend Appointments Available?

Visits are typically made during standard business hours, Monday thru Friday from 8:30-4pm.

How Do I Make an Appointment?

Click the button, “Book Appointment.” You will then be directed to our website to book an appointment or you can call our office to book an appointment.

What is Needed Prior to my Appointment?

You will need to complete all new patient forms and consents. You’ll also need to provide your insurance card to verify eligibility. Other documents that you need to get ready before your visit are identification card, insurance card (if applicable), and medical records.

Do You Offer Telehealth Service?

Yes. We offer telehealth upon request or for urgent visits.

What if I Need an Urgent Appointment?

We will evaluate the urgent need and you may choose a Televisit which can be done within 24-48hrs of your request or a home visit in 3-5 business days.

What if I am Sick?

Call our office and request a sick call which can be scheduled within 24-48 hours. If it is a life-threatening emergency, please call 911 or go to the nearest emergency room.

What is Your Cancellation Policy?

We ask that you give at least a 24-hour notice before cancelling the televisit or house call visit.

How Often Will the Medical Provider Visit?

Your health care needs will determine house call frequency. If you are sick and experiencing issues, your Provider will see you more often. The Medical House Call Clinic Provider will create an individualized plan of care with you for overall health management.

Services & Support

What If I Need to See a Specialist?

 We will make referrals to the appropriate specialists based upon your individual medical needs. Please notify your specialist that we are your PCP so we can obtain your progress notes for collaboration of care. You can also sign a medical release so we can obtain these records.

How Do We Safely Manage Insecure Information?

MHCC is Health Insurance Portability and Accountability Act (HIPAA) compliant utilizing secure Electronic Medical Record that sets the standard for sensitive patient data protection along with secure billing. The technology services security measures in place to protect health information (PHI), physically, and in network to ensure processes are HIPAA compliant.

 

How Do I Get Help With the Patient Portal?

If you are having password or technical issues and are unable to log in to the patient portal, please call the office directly for immediate assistance.

What If I Need to Contact my Provider After Hours?

The on-call Provider is available after hours via phone to assist you with urgent requests that cannot wait until the next day. However, if it is a life-threatening emergency, please call 911 or go to the emergency room.
